Age 26 and The “Opt-Out” Form
UnitedHealthcare has issued a thorough FAQ geared toward answering your questions on dependent coverage, college
graduate eligibility, and re-enrollment procedures up until implementation of the PPACA on Sept. 23, 2010.
Moreover, in an effort to work with customers not wishing to extend existing coverage for college students, an
“Opt-Out” Form will be mailed to all fully insured customers beginning May 5th.

Please note that employers must have the opt-out form completed and submitted to UnitedHealthcare by May 14,
2010 to exclude their plan from this program. No action is required if you wish for your plan to
participate in this program.
