UHc HB 2015 Information
Do you know what information is needed for UHc to use for your House Bill 2015 cases?
- Aggregate paid claims experience by month (medical, dental and Rx)
- Total premium paid by month
- Total number of covered employees by month (by coverage tier)
- Total dollar amount of claims pending
- Individual claims reports for members with $15,000+ in claims in preceding 12-month period, including:
- Unique identifying number
- Amounts paid
- Dates of service
- Procedure and diagnosis codes
- Pre-certification requests in preceding 30 days for hospital stays of 5 days or more
- Additional request can be made for those individuals on the individual claims report:
- Prognosis or recovery information
- Most recent case management information (not defined by statute)
- Future expected costs and treatment plan
